Every resident has their own Individualized Care Plan (ICP). This is developed through the initial assessment evaluation performed by our Wellness Team prior to each move-in. Second Care Plan review is completed in fourteen days, then quarterly unless there is a change in condition. This is to ensure every resident is receiving the proper care according to their needs. Families and residents are involved with each Care Plan review.
Families can experience a change in pricing when there is a change of condition. There are different levels for the Care Plan, Medications and our Continence Program. Also, the annual suite increase. Families will be notified and informed after all Care Plan reviews; discussion of fiscal increase will be had at that time. Thirty-day written notification for suite changes when applicable.
Our professional chefs will work with you and your loved one to adjust meals to meet most dietary needs and restrictions; It is also vital that we know of all food allergies. Per doctor’s order some require pureed, mechanical soft and or thickened liquids, low sodium and sugar free. These options are also provided with desserts.
No, we do not provide transportation to medical appointments. If you need specialized medical transportation, please work with our team in advance to determine the best methods available to you.
